Unlicensed drugs must be used in accordance with the Trust's policy http://www.ekhuft.nhs.uk/staff/clinical/drugs-and-therapeutics-committee/policies-and-guidelines/guidelines/ This is to remind the prescriber that special prescribing arrangements may be necessary. For example a general practitioner is unlikely to accept a recommendation to use an unlicensed drug without a full understanding. The cost of an unlicensed drug prescription to the NHS is likely to be higher for a prescription written on an FP10 form than if written as a Trust internal prescription, sometimes over ten times the cost. Unlicensed drugs and specials not on the formulary are likely to have to be repeat prescribed by the Trust clinician initiating such treatment. Unlicensed specials do not have a NHS list price and as such we have no control over price. Price varies between suppliers and often is very high. Please can you make sure all prescribers within your speciality are aware of this and before prescribing any unlicensed drug they review use of the unlicensed drug- they should only be used if there is no other suitable drug.  Any unlicensed drug should be on the formulary. Supply should be through the hospital pharmacy.Pharmacy will find the most cost effective source of the drug. The  patient can take the script to any of the main site hospital pharmacies, but please make clear these drugs are bought to order and will not be available to collect at once. All patient details including address and telephone number should be on the script. Please explain to the patient what you are doing and the pharmacy  will be in touch with the patient. We will either post to the patient or it will be available to collect from one of the three main sites. Most drugs will be available within 10 days

      Warning The drug is only given weekly in its immunosuppressive indications. Internal drug charts must be scored out on the days it is not to be administered. Those administering the drug on an internal script must first check that the script has been endorsed as safe by a pharmacist. Exceptionally out of hours administration is possible on scripts endorsed by a ST3 level or above trainee in GIM or specialist physician. Note that inappropriate administration of daily oral methotrexate (even without harm) using an electronic prescribing/administration system is regarded as an NHS Never Event.
      Warning Withhold during antibiotic courses and in acute kidney injury.

      • Consultant initiation only
      • Patients should have a methotrexate diary
      • Methotrexate is available as 2.5mg and 10mg tablets. This is a potential cause of confusion and has led to patients taking accidental overdoses. To avoid this only 2.5mg tablets should be prescribed and specified on the prescription.
         
          F2 doctors can only prescribe or transcribe for immunosuppression if competency formally established and signed off. All other doctors may only prescribe having competency for drug and route in that indication.

    Use as per Malaria Guidelines

  • Severe or potentially complicated malaria
    • Artesunate regimen below is appropriate for adults only on expert advice.
    • Dose: 2.4 mg/kg given as an intravenous injection (bolus) at 0, 12 and 24 h then daily thereafter until patients recover or parasites clear. See below for information about supply.
    • A 7-day course of doxycycline should also be given.

    Unlicensed Drug Artesunate is not licensed in the UK but can be made available from specialist tropical disease centres in London – phone Hospital for Tropical Diseases on 0845 1555000 and ask for the infectious diseases (ID) registrar on call. Out trust only stocks in Emergency Drug Cupboards enough drug to initiate treatment so please make sure initiating centre is aware that we may need to obtain extra stocks and contact on-call pharmacist to check sufficient stocks in Trust for full treatment
  • Artesunate should be considered especially in patients with
    • severe malaria from Asia (where there is evidence of relative quinine resistance)
    • hyperparasitaemia (>10% red cells parasitised)
      a history of cardiac arrhythmia, or risks of arrhythmia.

  • Nitrofurantoin is contraindicated in patients with an estimated glomerular filtration rate (eGFR) of less than 45 ml/min. However, a short course (3 to 7 days) may be used with caution in certain patients with an eGFR of 30 to 44 ml/min. Only prescribe to such patients to treat lower urinary tract infection with suspected or proven multidrug resistant pathogens when the benefits of nitrofurantoin are considered to outweigh the risks of side effects.
